The most common mistake is giving both people equal size and equal weight, which leaves the eye with nowhere to go. Decide who the draw is for this episode and make them larger, closer, or better lit. If the guest is the reason people click, the guest dominates the frame.
If your guest is recognisable to your audience, their name is the single strongest element you can put on the thumbnail. If they are not, the name is wasted space and the topic should take it instead.
"Interview with a neuroscientist" is a topic. "You are sleeping wrong" is a claim. Podcast thumbnails that carry a specific, contestable statement outperform ones that describe the subject.
Short-form podcast clips live or die on the face. A reaction mid-sentence, mid-laugh or mid-disagreement reads as a real moment. A posed studio shot reads as an advert.
Two people in chairs with microphones, shot wide, is the most common podcast thumbnail and the least effective. Crop in. The microphones tell the viewer it is a podcast, and they already knew that.

1280 x 720 pixels, 16:9, under 2MB, in JPG, PNG, GIF or WEBP. That is the same for every niche. Design at that size from the first click rather than resizing at the end, because resizing a finished design moves text you placed carefully.
Faces are the whole format. Expression carries clips especially: a reaction mid-sentence, mid-laugh or mid-disagreement reads as a real moment, where a posed studio shot reads as an advert.
Restrained, with one strong accent. Podcast thumbnails drift towards heavy graphic treatment with bars, frames and gradients competing for attention. The face and the claim carry this format, so the palette can stay quiet and let them work. A single accent colour used consistently across episodes also makes your channel recognisable in a subscription feed.
Use your own recordings and your own set. If a guest appears, you need their permission for the likeness, which is usually covered by the recording agreement but worth confirming before you publish.
Only when the name sells it. If your guest is recognisable to your audience, their name is the strongest single element you can use. If they are not, the name is wasted space and the topic or the claim should take it instead.
Three to five at most. The thumbnail is seen at around 168 pixels wide on a phone, and longer text at that size becomes a grey smudge. If the words repeat what the image already shows, delete them and let the picture work.
